By registering for a course, I am acknowledging this release and waiver executed on the date of registration between myself and 4th Box Training and Consulting LLC and the entity providing range services and its/their agents, servants, employees, directors, officers, and landlords, hereinafter referred to as “4th Box Training” and “The Range”.

 

I hereby acknowledge that firearms and shooting activities require extreme care and carries with it the potential for death, serious injury, and property loss. I hereby assume the risk of use of the shooting range and any other facilities provided by 4th Box Training for conducting firearms training.

 

I certify that I am physically and mentally competent to use the range and facilities, to handle firearms in a safe manner and to enter into this agreement. 

 

In consideration of permission granted to me by 4th Box Training and The Range to enter and use the shooting range, I hereby take action for myself, my executors, spouse, administrators, heirs, next of kin, successors and assigns as follows:                

Waiver, Release, Discharge, and agree Not to Sue for any and all liability from any death, disability, personal injury, property damage, property theft, or any and every claim, demand, action or right of action of whatever kind of nature, either in law or equity which may hereafter accrue to me as a result of my use of the range or premises, or participation in any activities sponsored by or Related to 4th Box Training and The Range its/their agents, servants, employees, directors, officers, landlords, representative(s) and agent(s) and volunteer and Indemnify and Hold Harmless the person and entities mentioned in this agreement from any and all liabilities or claims made by other individuals or entities or for my actions at the range or premises or in any event or activity sponsored by or associated with 4th Box Training and Consulting LLC or The Range.

 

I hereby give my consent to receive treatment in the event of my injury, accident and or illness which occurs at the range or premises or during any 4th Box Training and range activity.

 

I hereby certify that I am not under indictment in any court for a crime punishable by imprisonment for a term of one year or more, that I have never been convicted in any country of a crime punishable for a term of one year or more, that I am not a fugitive from justice; that I am not a habitual drunkard or an unlawful user of, or addicted to any illegal or prescription drug substance to include marijuana, depressants or stimulants or any other narcotic drug; that I have not been discharged from the Armed Forces under dishonorable conditions; that I am not an alien illegally in the United States; and that I am not a person, who having been a citizen of the United States, who has renounced their citizenship.

 

Further, I acknowledge that I have seen and read the respective Range’s instructions/rules and understand and agree to comply with these range instructions/rules and to follow any additional instructions, written or oral, that I may receive from the Range Master or other persons present and in authority representing 4th Box Training and Consulting LLC and the range.

 

I hereby certify that I am 18 years of age or older, and I have read this document and understand its contents and am entering into a legally binding agreement, and I give my full consent for images, audio, or video media of me that is captured during any training I attended, to be used by 4th Box Training and Consulting LLC for the purposes of advertising and marketing.
